The song "Leoric" from Diablo 2 ranks pretty up there in evilness to me.

    Video which plays that song:

Just about anything from Silent Hill and the original Baroque (though Voice of the Abyss from the remake is pretty damn creepy).

Dr. Gero's Theme (Androids Extended) from Bruce Faulconer's Dragon Ball Z soundtrack.
"Personally, I prefer the tranquility of my personal retreat on Byss, which has many lake chains that are not unlike those on Naboo. I also believe in encouraging the growth and development of some life-forms, if only to enjoy the eventual pleasure of guiding them toward their deaths. The only negative aspect of Byss is a relatively small population and a certain lack of culture." - Palpatine

Can't believe nobody suggested Vagrant Story. It's like 55 tracks and only about five have anything really bright in them. I'm not the most knowledgeable person, but it's probably the single most unrelentingly dark game soundtrack outside of the horror genre.

For me, I think it was the bgm of Haunted House from Super Mario 64 gave me chills from a game for the first time back then. Other's are
Bowser's Castle from Mario Kart 64
    Luigi Mansion's various bgm
    Ocarina of Time's Shadow Temple
Majora's Mask's Ikana Valley/Old Castle and Rockvale Temple
    Super Mario World's Haunted House
Twilight Princess's Armogohma Battle First Half (Still creeps me out, but cannot denie to listen to it)
GCN's Resident Evil remake, stopped playing 1/3 on the way. The music scares the crap out of me, like it was toying with my sanity. Scariest bgm that I've heard so far from a game...

Surprised no one mentioned this one (creepy but one of my all time favorites):

You can't get much creepier than Shooting Stars and Funeral Theme from Monster Rancher 2 (PS1). I used to get nightmares after a monster died on the game just because of the music.

And who wouldn't get creeped out? This scene just comes at you out of nowhere. o_O

Oh and the level "Computer Station" had music that sounded like the noise my toilet made on a windy day, scared me to death when I was a kid

To me Resident Evil probably has the creepiest music ever. The mansion kitchen level in the original is supremely freaky. Some of the later music is dark without being creepy (still great, though, #4 has some of my favorite VG music of all time). Silent Hill has some disturbing stuff (I generally prefer Broken Notes to the original music, though), and Dead Space has some great tracks.
